FRCPath from Pakistan — The Complete Guide for Pakistani Pathologists

You're an FCPS-trained pathologist in Lahore, Karachi, Islamabad, or Rawalpindi. You're good at what you do. But you want something more — a credential that opens doors to the UK, the Gulf, and the world. FRCPath is that credential. It's recognised by PM&DC. Pakistani doctors have been sitting it since 2011. And you can take Part 1 from your home in Pakistan. This guide covers everything — written by pathologists who've helped Pakistani candidates clear the exam.

FRCPath Is Officially Recognised by PM&DC (Pakistan Medical & Dental Council)

FRCPath is listed among recognised UK postgraduate qualifications by PM&DC — alongside MRCP, FRCS, and FRCR. Holders can register the qualification and practise as consultant pathologists in Pakistan.

Can You Take FRCPath from Pakistan?

Yes — and Pakistan has a deeper connection to FRCPath than most countries.

Pakistan is one of the few countries with a documented history of hosting FRCPath examinations on its soil. Here's what you need to know:

Part 1 — Can be taken from Pakistan

FRCPath Part 1 is currently run online and can be sat from home, anywhere in the world — including Pakistan. You sit the exam on a secure digital platform (RISR, changed from Testreach from Autumn 2025 onwards) with remote proctoring. You need a stable internet connection, a quiet room, a webcam, and a computer that meets the technical requirements.

For Autumn 2026, RCPath has stated their aim is to deliver examinations in test centres with in-person invigilators both in the UK and internationally — which could potentially include Pakistani centres, given the historical precedent.

Historical context: Since 2011, the Royal College of Pathologists established an examination centre at the Armed Forces Institute of Pathology (AFIP) in Rawalpindi for conducting FRCPath Part 1 examinations. This was a significant development that gave Pakistani candidates local access to the exam. The current online format means you no longer need to travel even to AFIP — you can sit Part 1 from your home.

Part 2 — Requires travel, but Dubai is an option

Part 2 Histopathology can potentially be taken in Dubai (UAE), Cairo (Egypt), or Irbid (Jordan) — subject to sufficient candidates each session. These overseas centres are offered on an annual basis, either in Spring or Autumn. Dubai is typically the most accessible overseas centre for Pakistani candidates — direct flights from Karachi, Lahore, and Islamabad are frequent and relatively affordable.

If no overseas centre is available for your session, you will need to travel to the UK.

The CPSP–RCPath connection

The Royal College of Pathologists has a formal partnership with the College of Physicians and Surgeons Pakistan (CPSP) and the Pakistan Association of Pathologists (PAP). This institutional relationship means RCPath understands and values Pakistani pathology training. Your FCPS Histopathology training is well-regarded as preparation for FRCPath.

What Pakistani candidates should know Pakistan has 29 CPSP-accredited centres for FCPS Histopathology training across the country. The structured nature of FCPS training — with its emphasis on surgical pathology, cytopathology, and clinical rotations — provides a strong foundation for FRCPath Part 1. Many Pakistani candidates who have cleared FRCPath report that their FCPS training covered the majority of the syllabus. The key additions are UK-specific guidelines (NICE, RCPath datasets) and familiarity with the EMQ question format.

PKR conversions are approximate based on mid-2026 exchange rates. Actual salaries vary by institution, city, experience, and negotiation.

The math is simple A senior FCPS consultant in Karachi or Lahore earns approximately PKR 250,000–400,000/month at the peak of their career. An FRCPath consultant in the NHS starts at £88,364/year — roughly PKR 5.3 lakh/month at current exchange rates, with progression to £119,113 (PKR 7.1 lakh/month). In the Gulf, salaries of AED 40,000–70,000/month are common and entirely tax-free. The total investment in FRCPath (approximately PKR 12–18 lakh for both parts including travel) is recovered within the first 2–3 months of a UK or Gulf consultant position.

How to Apply for FRCPath from Pakistan — Step by Step

The process is straightforward. Here's exactly what to do.

1

Create an account on rcpath.org

Visit rcpath.org and register as a candidate. You'll need your passport details, FCPS certificate information, and training history.

2

Check the application window

Applications open twice a year. Spring: applications typically open late November, close early January. Autumn: applications typically open May, close early July. For Autumn 2026, applications close Friday 3 July 2026.

3

Submit your application with supporting documents

Upload your FCPS certificate (or provisional pass letter), passport, and training documentation. Select Histopathology as your specialty.

4

Pay the exam fee: £673 (~PKR 240,000)

Payment is online by international card. The fee is the same for UK and overseas candidates. Additional RCPath registration fees may apply for first-time candidates.

5

Complete the technical practice test

RCPath sends a practice test to verify your computer setup, internet connection, and webcam. Do this seriously — Pakistani internet can be unstable, so test from the exact location where you'll sit the exam.

6

Sit the exam from home

On exam day, log in to the RISR platform. The exam is timed and remotely proctored. Ensure a quiet room, stable internet (consider a backup mobile hotspot), no interruptions, and your webcam is working.

7

Wait for results (~6-8 weeks)

Results are emailed to candidates by midday UK time on the scheduled results date. A detailed performance letter follows in the subsequent week.

Autumn 2026 deadline approaching: Applications for the Autumn 2026 sitting close on Friday 3 July 2026. Late applications are not accepted. This is also the last sitting in the current exam format — the new format begins Spring 2027.

Common Challenges Pakistani Candidates Face — And How to Overcome Them

Real problems that Pakistani doctors encounter, with practical solutions.

1. Internet stability during the online exam

Pakistan's internet infrastructure can be unreliable, and this is the single biggest practical concern for online examinations. The RCPath exam platform (RISR) requires a stable connection throughout the 3-hour exam. A disconnection could result in lost time or even disqualification.

Solution: Test your internet speed at the exact time and location you plan to sit the exam — speeds can vary by time of day. Arrange a backup mobile data hotspot (Jazz, Zong, or Telenor 4G) as a failover. Consider sitting from a location with reliable fibre optic internet (available in major cities like Lahore, Karachi, Islamabad). Some candidates book a quiet hotel room with guaranteed high-speed internet for exam day.

2. Currency and payment

The exam fee is £673, which is approximately PKR 240,000 at current exchange rates. International card payments can sometimes face issues from Pakistani banks due to foreign currency restrictions.

Solution: Use an international Visa/Mastercard that allows GBP transactions. Some Pakistani banks (HBL, MCB, UBL) offer cards with better international transaction success rates. Alternatively, consider a freelancer/digital banking card that supports international payments.

3. UK-specific guidelines and clinical practice

FRCPath tests UK clinical practice — NICE guidelines, RCPath datasets for histopathological reporting, UK clinical governance frameworks. Pakistani training follows its own clinical standards, which may differ in some areas.

Solution: Allocate dedicated study time (2-3 weeks) specifically for UK guidelines. Download RCPath tissue pathways and datasets from rcpath.org. Our FRCPath Part 1 course specifically covers UK-relevant guidelines that international candidates need.

4. EMQ format unfamiliarity

FRCPath uses Extended Matching Questions (EMQs) alongside Single Best Answers (SBAs). Pakistani FCPS exams primarily use MCQs and clinical cases. EMQs test broader differential diagnosis thinking and can be challenging for candidates unfamiliar with the format.

Solution: Practice with EMQ-format questions extensively. Our mock test bank of 800+ questions includes EMQ-pattern questions designed for FRCPath preparation.

5. Isolation — no local study group or mentorship

Unlike UK trainees who have departmental teaching, study groups, and educational supervisors, Pakistani candidates often prepare in isolation. This can lead to knowledge gaps, unfocused study, and difficulty assessing readiness.

Solution: Join a structured preparation course with live sessions and peer interaction. Our FRCPath Part 1 course includes weekly live sessions, expert mentorship from Dr. Maitrayee Roy (MD FRCPath) and Dr. Akshay Bali (MD DipRCPath), and a WhatsApp community of fellow candidates from Pakistan, India, and the Gulf.

6. Part 2 travel logistics

Part 2 requires travel — either to the UK or Dubai. For Pakistani passport holders, obtaining a UK visa requires advance planning.

Solution: Start visa planning 3-4 months before Part 2. A Standard Visitor visa allows you to sit an exam in the UK. If Dubai is offered as a Part 2 centre, Pakistani nationals can obtain a UAE visa on arrival or through a straightforward e-visa process — this is often the easier option. Many Pakistani candidates combine the Part 2 trip with hospital observerships or short courses in the UK.

FCPS to FRCPath — How Your Pakistani Training Prepares You

The syllabus overlap is substantial. Here's what matches and what doesn't.

Organ-system Surgical Pathology (all systems)✓ Yes — FCPS Part II focus✓ YesMinimal
WHO 5th Edition Tumour Classifications◐ Partially (depends on centre)✓ Yes — heavily testedUpdate needed
IHC Markers & Molecular Pathology◐ Partially✓ YesNeeds focused study
Cytopathology✓ Yes — FCPS rotation✓ YesMinimal

The bottom line: if you've completed FCPS Histopathology, you have approximately 70-80% of the FRCPath Part 1 knowledge base. The gaps are specific and addressable — WHO 5th Edition updates, molecular pathology depth, UK-specific guidelines, and EMQ format practice. A structured 6-month preparation plan can close these gaps efficiently.
